The Portland Interscholastic League Sports Hall of Fame Selection Committee is presently accepting nominations for the Fall 2023 Induction Banquet. Nominations must be received on or before January 1 of the year of induction. Nominations received after this date will be considered for the following year.

The PDF is a "fillable" form which can be saved on your computer to either forward as an email attachment, or print out and mail to us at the address below. You may also print out the empty form and fill it in, legibly, by hand.

Once received, the form will be delivered to the proper school representative or the committee chairperson to start the process. Please fill in as much information as possible so the selection committee can start its research immediately. For more information, please see our nomination criteria.
